 Community Development and Social Work Officer

Description

PURPOSE OF THE JOB:
The Community Development & Social Work Officer plays a key role in bridging Ubuntu Life Foundation with the community through holistic social support, empowerment, and inclusive development initiatives. The officer supports children, families, and clients through home visits, assessments, and follow-ups, while also designing and implementing community empowerment projects, parent support programs, and health awareness campaigns to strengthen resilience and promote inclusion.

KEY TASKS AND RESPONSIBILITIES

  1. Client Support & Case Management
    1. Identify children, families, and community members in need of social and economic support.
    2. Conduct client assessments to determine strengths, challenges, and support systems.
    3. Develop individualized care or intervention plans for clients, ensuring follow-up and measurable progress.
    4. Maintain up-to-date case files, home visit reports, and recommendations.
  2. Home Visits & Assessments
    1. Conduct home background checks and follow-up visits for school, therapy, and neuro clients.
    2. Assess socioeconomic conditions and provide feedback to inform support interventions.
    3. Refer children and families to Ubuntu Life services and monitor their adjustment and progress.
  3. Community Empowerment & Development
    1. Identify community needs and initiate empowerment projects to improve the livelihoods of parents and caregivers.
    2. Organize and facilitate capacity-building sessions, economic empowerment activities, and community group meetings.
    3. Build partnerships with local organizations and stakeholders to enhance community development initiatives.
  4. Health & Social Outreach
    1. Conduct health talks and awareness campaigns on disability inclusion, therapy, epilepsy, and child welfare.
    2. Support clinical and therapy outreach programs and facilitate active case finding within the community.
    3. Work closely with the clinic, therapy, and education teams to ensure coordinated follow-up care.
  5. Advocacy & Resource Linkage
    1. Advocate for the rights of children with disabilities and their families within the community.
    2. Link families to available social support programs such as NHIF, NCPWD registration, and other relevant community resources.
    3. Collaborate with local authorities and NGOs to enhance support for special needs children and families.
  6. Monitoring, Evaluation, and Reporting
    1. Track progress and outcomes of interventions and empowerment projects.
    2. Prepare weekly and monthly reports on activities, impact, and lessons learned.
    3. Document success stories, case studies, and community impact narratives for visibility and accountability.
  7. General Duties
    1. Support parents and caregivers in coping with challenges related to raising children with special needs.
    2. Participate in staff meetings, trainings, and cross-departmental collaborations.
    3. Perform any other duties as assigned by the supervisor.

Minimum Qualifications

  • Bachelor’s degree or Diploma in Social Work and Community Development, Sociology, or a related field.
  • Minimum 2 years’ experience in community engagement, social work, or similar roles.
  • Experience working with children with special needs or disability inclusion programs is an added advantage.
  • Excellent interpersonal, communication, and organizational skills.
