JOB PURPOSE
The Field Officer works closely with the HTS team and other staff serving epidemiologically at-Risk Populations, and reports to the coordinator. With the support of the coordinator, he will coordinate field related activities with the Ministry of Health (MOH) Officers and other stakeholders at the County and Sub-County levels. He/ She will Act as a liaison person between the community and the program team.
DUTIES AND RESPONSIBILITIES
- Provide overall coordination of project outreach activities within your area of coverage.
- Ensure the project adheres to the national guidelines during service provision and maintains quality.
- Ensure outreaches are integrated to reach a minimum package of services to beneficiaries
- Provide coordination of peer education recruitment and ensure good representation of PEs in the project sites.
- Facilitate mobilization and enrolment of epidemiologically at-risk populations for service uptake.
- Supports HTS team to ensure all HIV positive Clients are put on treatment.
- Coordinate and/or facilitate health education sessions to target populations.
- Facilitate the provision of structural interventions, linkage to social services and promote alternative livelihoods for clients.
- Make appropriate referrals for on-going support e.g. Home-Based Care, PMTCT, CCC and TB Clinics. This includes follow up/tracking of the referral cases.
- Ensure documentation of all relevant community tools and registers
- Documentation, management and reporting of commodities used.
- Captures and shares success stories, lessons learnt, and challenges promptly to inform program decisions.
- Compile and submit timely reports to the supervisor and in the correct templates
- Participate in routine data quality assessments monitoring and quality improvement activities from time to time
Qualifications
Education and Experience
- Diploma Holder in Nursing, Counselling Psychology, Community Health, public health, Social Sciences or other related fields.
- Holder of a NASCOP HTS Certificate is preferred.
- Training in Peer Education, Behaviour Change Communication or HIV & AIDS is added advantage
- Two years relevant work experience in HIV prevention, care and treatment, more so with epidemiologically at-risk populations, PrEP and Post response is desirable.
- Experience working in Arid and Semi-Arid regions is an added advantage

- Excellent interpersonal skills
