Medical Social Worker

Job Summary
Responsible for providing psychosocial support to patients and families, helping address social and practical challenges affecting patient care, and linking patients with appropriate support services.
Key Responsibilities
- Assess patients' social and psychosocial needs.
- Provide counseling and supportive interventions within professional scope.
- Assist patients and families facing social, financial, emotional, or practical challenges.
- Facilitate appropriate referrals to internal and external support services.
- Support patients with discharge and continuity-of-care planning.
- Advocate for vulnerable patients where appropriate.
- Work closely with doctors, nurses, and other healthcare professionals.
- Maintain accurate and confidential social-work records.
- Participate in community outreach and health-promotion activities.
- Support safeguarding and protection of vulnerable patients.
- Follow professional ethics, confidentiality, and applicable healthcare policies.
Requirements
- Bachelor's degree or relevant recognized qualification in Social Work.
- Appropriate professional registration where applicable.
- Experience in a healthcare or community setting is an advantage.
- Strong counseling, communication, and interpersonal skills.
- Compassionate and patient-centered approach.
- Ability to work with patients from diverse backgrounds.
