Project Description
Tujenge Jamii is an award led by Deloitte and Touche LLP. The core vision of Tujenge Jamii is to create a county owned, county led, and county managed integrated platform for delivery of life saving HIV, Women & Children, Water Sanitation and Hygiene (WASH) and Nutrition services, with close collaboration between health facilities and communities and in line with the GHSD PEPFAR limited waiver guidance. The project is working in Baringo, Kajiado, Laikipia, Nakuru & Samburu counties: In Nakuru county, the project is implementing HIV prevention, Care and Treatment Services, Women’s & Children, WASH and Nutrition, while in Baringo, Kajiado, Laikipia and Samburu counties, the project is implementing HIV prevention, Care and Treat interventions.
Tujenge Jamii’s objectives are to:
- Increase access and demand for quality HIV prevention services.
- Increase access and demand for quality HIV treatment services; and
- Increase access and demand for quality Women’s & Children, nutrition, and WASH services.
Position Summary
Working collaboratively with the MOH facility team, facility in-charge, CLI and TJ mentorship team, TJ Monitoring and Evaluation Officer with close supervision of the TJ Snr. M&E Associate, the Health Records Information Officer (HRIO) will be responsible for mentoring facility health care workers on proper documentation of NASCOP MOH HIV service registers, conducting regular data quality assessments including data verification, ensuring daily, weekly and monthly submission of accurate facility reports on time, coordinate facility data review meetings and support EMR and other HIS products implementation. S/he will be required to maintain confidentiality and integrity for the facility data as directed and in line with the National Health Information Policy and Guidelines.
Duties and Key Responsibilities.
- Conduct regular facility-level NASCOP MOH and program M&E tools assessments in collaboration with facility staff and the in-charge to ensure consistent availability of standard tools for documentation and reporting across all service delivery points, including OPD and integrated HIV services (PMTCT, TB clinic, laboratory, pharmacy, among others).
- Closely monitor NASCOP MOH and program M&E tools usage, projection, and forecasting at the facility level and provide timely updates to the CLI (Sub-County Health Records Officer) and TJ Snr. M&E Associate to ensure continuous availability of tools across departments.
- Provide continuous targeted mentorship to healthcare providers on correct use and proper documentation of standard National NASCOP MOH and program M&E tools across all service areas, including OPD and integrated HIV services i.e. PMTCT, TB Clinic, Pharmacy, Laboratory among others.
- Support and coordinate timely and accurate data collection, reporting, and submission processes (including all MOH and program reports) across all integrated service delivery points at the facility.
- Work with facility staff and the in-charge in the preparation and compilation of accurate MOH 731, program Form 1a, and HCA reports, and ensure timely submission to the Sub-County Health Records Office by the 5th of every month.
- In conjunction with the facility in-charge and other staff, coordinate and actively participate in regular data review meetings with documented minutes to promote a culture of data demand and information use for decision-making across all service delivery points, including OPD and integrated HIV services (PMTCT, TB Clinic, Pharmacy, Laboratory among others) ensuring that Performance Monitoring Charts are updated.
- Utilize the CLI Dashboard to review data across all integrated service delivery points and ensure surge reports are uploaded by 6:00 PM daily.
- Serve as the facility EMR focal person and champion by supporting TaifaCare powered by DHA implementation and other HIS products (e.g., eHTS, case-based surveillance, NASCOP Dashboard, Data Extractor, Ushauri, Nishauri, among others) through user mentorship and generation of ad-hoc data reports.
- Ensure timely EMR backup and submission to the National Data Warehouse (NDWH) with all relevant dockets via DWAPI by the 10th of every month and ensure weekly submission of case-based surveillance data.
- Working closely with the CLI team and TJ Snr. M&E Associate, coordinate routine data quality activities including data verification, EMR data quality assessments, and data triangulation across service delivery points, and support implementation of data quality improvement and corrective action plans.
- Work closely with the facility Multidisciplinary team (MDT) to conduct routine program performance reviews (including patient outcomes, retention, and service uptake), identify service and data gaps, and ensure appropriate corrective actions are implemented across all integrated services.
- Ensure that findings and recommendations documented in the MOH supervision booklet during CLI and program supervision visits are actively followed by the respective facility teams.
- Maintain an organized and up-to-date inventory of all NASCOP MOH paper-based tools across departments, as well as EMR and other ICT equipment at the facility.
- With support from the CLI (SCHRIO) and TJ Snr. M&E Associate, coordinate dissemination of site-level targets (including PEPFAR DATIM and MOH indicators) and ensure visibility at key service delivery points.
- Participate in continuous quality improvement (CQI) activities through CQI and work improvement teams (WIT) to strengthen service delivery and data use across all integrated departments.
- Ensure confidentiality and security of patient data across all service points by enforcing proper storage of registers and secure use of EMR systems.
Minimum Qualifications and Requirements for Internal Candidates
- Diploma in Health Records and Information Technology/ with at least 2 years work experience in a busy CCC Department in a high-volume health facility.
- Good understanding and familiarity with NASCOP MOH HIV M&E tools is a must.
- Demonstrated experience with National MOH Health Management Information Systems (KHIS, NDW, NASCOP VL /EID Website) and Electronic Medical Record (EMR) system (TaifaCare).
- Good personal attributes: Good interpersonal skills such as team player, good organization, planning, well-coordinated and good communications skills. Good mentorship skills are an added advantage.
- Computer proficiency in MS Office Suite (MS Word, MS Excel & MS PowerPoint)
